CDC Health Equity Scholarship
Do you serve Priority Populations?
Priority populations include Medicare beneficiaries; men; Black or African American, Asian American, Hispanic, American Indian, Alaska Native, and Pacific Islander persons; and people with visual impairments or physical disabilities.
The J. Moss Foundation has been awarded with the CDC Health Equity Scholarship that allows our organization to train prospective Lifestyle Coaches AND seasoned Lifestyle Coaches that serve priority populations at no cost (Lifestyle Coach Training or Advanced Training).
We have a limited number of spots available for our upcoming virtual LSC training dates in June and August of 2023. Our Advanced Training courses are on-demand which allows us to register trainees based on a rolling basis.

Additional Requirements
Applications will be considered from CDC-recognized organizations with at least pending recognition. Scholarship support is allowed for an organization that has newly earned pending recognition, but has not yet trained their lifestyle coaches, provided their programmatic focus includes serving priority population(s).
In addition, per the scholarship application, “It is expected that trainees funded under this award mechanism will lead a cohort within 6 months of training completion.” Scholarship funds cannot be used by an organization that has not applied for CDC recognition.
If you are applying as a prospective Lifestyle Coach, you must work for a DPP recognized organization or your organization must have submitted the “Diabetes Prevention Recognition Program Application” within the last 15 days.

If you choose to apply for the Lifestyle Coach Training or the Advanced Training Scholarship you must Complete a 3 question post-survey upon completion of the Training (see attached form).
We will be selecting candidates on April 24th, 2023 via email.
